I am currently contracted with BCBS, Aetna, Medcost and UHC. Our office will assist you in determining the cost of care based on your plan.  

Payment

I accept cash, checks, and major credit cards as forms of payment.

Missed/Late Cancellation Appointment Policy

If you need to cancel or reschedule your appointment, please do so at least 24 hours in advance, so that we will have adequate time to offer the appointment to others.  Failure to do so will result in a late cancellation or missed appointment fee.
